Sign Up Instuctions

1. Access the FORMED Sign Up Page
2. Select "I am an Individual".
   
3. Register with your name, email address and valid monthly or annual subscription payment option.
   
   Note: Take note of the email address you use. It will be used to validate each sign in for future use.
   
   
4. The FORMED interface will launch and you're now ready to start viewing the content.
   
   Note: Once your account is created, you can select "Sign In", enter your email address. The access link will be sent to the registered email address to access FORMED.
